TERMS AND CONDITIONS

• This registration form is for the ESSK V-Class.
• Parents understand that ESSK will not be responsible for the supervision of their child while engaged in the ESSK V-Class.
• Parents represent that their home environment is safe during the ESSK V-Class.
• Parents will take responsibility for all cybersecurity issues while their child is on the internet participating in the ESSK V-Class.
• Only children who are registered in the ESSK V-Class may participate.
• There will be no sharing of personal information or outside contact between teacher and child during the ESSK V-Class.
• Parents give permission for the use of images from ESSK V-Class to be used for promotional purposes.
• Parents understand that any live sessions of ESSK V-Class will be taped for security purposes.
• Binding Arbitration – I agree that any dispute concerning, relating, arising out of or referring to the subject matter of this contract shall be resolved exclusively by binding arbitration in Suffolk County, New York, according to the then existing commercial rules of the American Arbitration Association and the substantive laws of the state of New York.
